Procurement-related Forms

These are the most common forms for both vendors and City departments and agencies.

• Disadvantaged Business Enterprise (DBE)
  Visit the Office of Supplier Diversity for all DBE information.
• Tax Clearance Form
  Form to receive clearance from delinquencies on City taxes
•  Affidavit of Compliance With Hiring Requirements                                                          Form in which a respondent may attest to their compliance with the City of New Orleans' hiring requirements in connection with proposed work to be performed for the City.
• Identification of Subcontractors                          Form to identify the persons the respondent expects to perform work as a subcontractor in connection with the respondent's proposed work to be performed for the City.
• Sample Professional Services Agreement                A sample of a professional services agreement which is substantially similar to the contract the selected respondents shall expect to execute in connection with the award of an Request for Proposals (RFP).

Executive Orders
