What is an Affidavit?

An affidavit is a sworn written statement that legally confirms the details of your auto theft report. By signing this document in front of a notary public, you're officially declaring that the information about your stolen vehicle is true and accurate.

Required Documentation

After reporting your stolen vehicle, you must:

1. Complete the auto theft affidavit form
2. Have it signed and stamped by a notary public
3. Submit within 7 days to NOPD
