Missing Person Affidavit (Form 007)

This document is the "National Crime Information Center (NCIC) Affidavit to Missing Person Report" (FBI/N.C.I.C. Form 007) used by the New Orleans Police Department.

A person completing this form would find:

  1. Legal Declaration: This is an affidavit (a sworn statement) where the person reporting someone missing legally attests that:
    • The information they've provided to NOPD is true and correct
    • They accept full responsibility for actions taken by NOPD in efforts to locate the missing person
  2. Required Information Fields:
    • Reporting person's name
    • Their relationship to the missing person
    • The missing person's complete name
  3. Special Notation: The form includes an important note that a valid physician's certificate must be attached if the missing person has a mental or physical disability. This is a specific requirement for certain types of NCIC entries.
  4. Verification Section:
    • Space for a witness name and signature
    • Fields for the reporting person to sign and print their name
    • NOPD Item # (case reference number)
    • Date and time fields
