To Apply, You Must Have the Following Documents

  • Birth Certificate
  • Social Security Card
  • Photo ID
  • Proof of Income (3 Check Stubs from 6 months prior to applying for services; Social Security Award Letter, Food Stamps and/or Welfare Award Letter - TANF)
  • Proof of Address (i.e. Utility Bill, Rent Receipt, State ID or Driver's License)
  • Current School Report Card (In-School Youth only)
  • Diploma / Withdraw Slip / Drop Notice (Out of School Youth only)
  • Parent / Legal Guardian Signature (Youth 18 Years and Younger)
