File Officer Commendations or Complaints (Form 230)

This is a form to be completed by a subject making either a commendation or complaint to the NOPD’s Public Integrity Bureau.

The Form Functions:

  1. Dual-Purpose System: The form is designed to document both positive feedback (commendations) and allegations of misconduct (complaints), creating a standardized intake process for either scenario.
  2. Comprehensive Documentation: It collects:
    • Complainant/reporting person information (contact details, demographics)
    • Incident specifics (date, time, location, NOPD item number)
    • Officer identification (name, rank, badge number)
    • Witness information
    • Details about arrests, citations, or injuries resulting from the incident
  3. Procedural Accountability: The form includes specific requirements for supervisors handling complaints:
    • Assigning a PIB CTN (Public Integrity Bureau Case Tracking Number)
    • Providing the complainant with a copy of the form and tracking number
    • Recording complainant interviews
    • Submitting the completed form before the end of their shift
  4. Narrative Documentation: Pages 2-3 provide space for detailed descriptions of:
    • The specific complaint and desired outcome
    • Officer actions deserving commendation
    • Minor violations requiring supervisor feedback
  5. Minor Violation Processing: The form includes a separate section for "Supervisor Feedback Log" entries for minor infractions, which requires:
    • Identification of the specific rule, paragraph, chapter, or general order violated
    • Signatures from issuing supervisors
    • Signature from the accused officer
    • Final approval/disapproval by PIB
