State and Local Partners

Through strong partnerships with state agencies and local organizations, we're building a more resilient New Orleans. Our network of dedicated partners works together to provide comprehensive support services, healthcare access, and community development programs. These collaborations enable us to better serve our residents and create lasting positive change in our community.

State Health Department.  Supports health services throughout the city.  Visit their website to learn more.

LPHI coordinates and manages public health programs and initiatives in the areas of health systems development and community health improvement and provides an array of services to help meet the needs of local and national partner organizations.

Health Guardians of Catholic Charities is an intensive and integrated care model that strives to remove and address any and all barriers (social, economic and health-related) to receiving effective health care.  Contact 504-310-6911 or through their website.

The LEAD program is a partnership between NOHD, New Orleans Police Department, the Vera Institute of Justice, and the Mayor’s Office of Criminal Justice Coordination.  LEAD is being piloted in the NOPD 8th District and gives officers the discretion to divert individuals at risk for arrest to case management instead of entering the criminal justice system.  Eligible participants are those who show signs or symptoms of mental illness or substance use disorder, and who are at risk of arrest for non-violent, low level Municipal offenses.  Case managers help connect participants to various resources, including mental health treatment, substance use treatment, and housing assistance.  Learn more at the New Orleans Health Dept (NOHD) Behavioral Health website.

One of Ryan White HIV/AIDS Program's primary functions is to identify qualified health care facilities and non-profit agencies to provide services that will decrease the rate of growth of HIV/AIDS within our community and increase the health and well-being of persons living with HIV/AIDS.  Learn more at the Ryan White website.

The Office of Community Development works to stabilize and revitalize communities in the City of New Orleans through the development of affordable housing and economic opportunity, and the delivery of community services that support Orleans Parish residents

LPCA represents 31 federally funded, private non-profit and public community health centers across Louisiana that serve more than 300,000 patients annually.  Learn more in this link

504HealthNet is an association comprised of 22 non-profit and governmental organizations in the Greater New Orleans area.  Our members independently operate 60 service locations ranging from comprehensive adult/pediatric primary care and behavioral health sites to medical mobile units.  Find a health center near you!

Since 1986 we have been the leading organization to call for comprehensive health care and secure housing for all.  We produce leading research in the field and provide the highest level of training and resources related to care for persons experiencing homelessness.  Learn more in this link.

The National Association of Community Health Centers (NACHC) organized in 1971. NACHC works with a network of state health center and primary care organizations to serve health centers in a variety of ways: research, training, education and partnership development.  Learn more in this link.
