  • Log in or register for new account
  • Select "UHC Rewards"
  • Follow the instructions provided to activate your UHC Rewards profile.
  • Follow the instructions provided to perform the UHC Rewards survey.
  • Complete all information and save.

Important: Both employee and spouse/domestic partner must complete survey

Annual Wellness Exam

  • Includes: Total Cholesterol, Glucose, and Blood Pressure tests
  • Must be coded as a "wellness visit" by your doctor
  • No co-payment required for wellness visits
  • Must be completed by October 31st to affect the next calendar year
