Wellness Program and Premium Discounts

  • Save on Your Healthcare Premiums
  • Annual savings: $180 to $360
  • All premium deductions are pre-tax
  • Program mandatory for premium discount eligibility

Who Must Participate?

  • Employees with health coverage
  • Covered spouses/domestic partners
  • Note: Not available for covered children (including adult children up to age 26)

Two Mandatory Requirements for Wellness Credit:

UHC Online Rally Health Survey Complete at www.myuhc.com: 

  • Log in or register for new account
  • Navigate to "My Account" > "Rewards"
  • Click "Finish Survey" under "Available"
  • Complete all information (can save and return later) Important: Both employee and spouse/domestic partner must complete survey

Annual Wellness Exam

  • Includes: Total Cholesterol, Glucose, and Blood Pressure tests
  • Must be coded as a "wellness visit" by your doctor
  • No co-payment required for wellness visits
  • Complete by October 31st of previous calendar year
