Healthcare Premiums and Wellness Credits

Overview: The United Healthcare Choice Plan Summary provides details on the City's healthcare plan, including medical, dental, and vision coverage. For specific questions, contact your department's Human Resource Manager/Personnel Officer or the CAO Benefits Administration Division.

Premium Structure

  1. The City pays a portion of employee healthcare insurance premiums.
  2. The remaining cost is deducted from the employee's paycheck.
  3. Contribution amounts vary based on factors like family size.

Leave Without Pay (LWOP) 

All employees on LWOP status are responsible for paying their healthcare premiums if they do not receive a paycheck.  In the event when an employee’s healthcare coverage is terminated for non-payment, they must wait until the first available Open Enrollment period to re-enroll into the City’s healthcare plan, and the coverage will not begin until January 1st of the following year.  All employees who are on LWOP status should contact the Benefits Administration Division to make payment arrangements.

Wellness Program

Our healthcare plan emphasizes employee wellness by offering premium discounts (wellness credits) for completing specific wellness activities.

Wellness Credits and Premium Reduction

  1. Default Rate: Employees who don't complete wellness activities pay the Non-Compliant Contribution rate.
  2. Reduced Rate: Complete ALL wellness activities to pay the lower Compliant Contribution rate.
  3. Spouse Coverage: • $15 credit when one covered adult completes activities • $30 credit when both complete activities

Important Notes:

  • Wellness activities are mandatory for premium reduction.
  • Each adult plan member (employee and covered spouse/domestic partner) must complete the activities.
