  • Expungement Form An expungement is the legal process of removing certain arrests, charges, or convictions from a person's criminal record. Use this form to petition the court for an expungement of eligible juvenile records.

 

  • Forma Pauperis Affidavit If you are unable to pay the required court fees due to financial hardship, you may be eligible to proceed "in forma pauperis." This affidavit is used to request a waiver of court fees.

 

  • Motion for Disclosure-Art 412 Form Article 412 of the Louisiana Children's Code governs the confidentiality of juvenile records. This form is used to request the disclosure of confidential information in accordance with the provisions of Article 412.

 

  • IJJIS-User-Agreement-Form The Integrated Juvenile Justice Information System (IJJIS) is a database used to track and manage juvenile cases. This user agreement form must be completed by authorized individuals seeking access to the IJJIS.
