Register Missing Persons Information (Form 212)

This is a form to be completed to collect information on a reported missing person for entry into the National Crime Information Center (NCIC) database.

The Form Items:

  1. Database Integration: It provides a standardized format for entering missing person records into the NCIC system, which is accessible to law enforcement agencies throughout the United States.
  2. Identification Fields: The form collects comprehensive identifying information including:
    • Complete name (last, first, middle)
    • Demographic data (sex, race, place of birth)
    • Date of birth and date of emancipation (if applicable)
    • Physical descriptors (height, weight, eye/hair color)
    • Unique identifying features (scars, marks, fingerprint classification)
  3. Identification Numbers: Fields for various identification documents and numbers:
    • Social Security Number
    • FBI Number
    • Driver's license information (number, state, expiration)
    • Bureau of Identification (B of I) Number
  4. Record Management: Administrative fields that facilitate tracking and management:
    • NCIC number and Message Key (MKE) code
    • Originating Agency Identifier (ORI)
    • Item Number/Originating Case Number (OCA)
    • Missing Person category code (DLC)
  5. Accountability: Documentation of both the reporting officer and the employee who enters the information into the NCIC system.
  6. Additional Information: A miscellaneous field for supplemental details that might aid in identification.

Each field on the form includes a standardized NCIC code (shown in parentheses), which ensures the information is properly formatted for the national database. This standardization enables immediate nationwide alerts and allows any law enforcement agency in the country to identify the missing person if encountered.
