EBO Compliance

The Office of Supplier Diversity is charged with monitoring and reporting DBE participation on City contracts to ensure DBE firms get their share of procurement opportunities. In addition to monitoring and reporting DBE participation, the OSD staff also:

• Performs evaluations of DBE utilization plans proposed by contractors;
• Reviews DBE participation waivers, substitutions, and modifications on City contracts; and
• Conducts compliance audits and site visits to monitor contract compliance.

• Download DBE Responsiveness Form (DBE Compliance Form-1)
  Use this form to identify the DBE subcontractor(s) in your bid, RFP, or RFQ. 
• Download Evidence of Good Faith Efforts (DBE Compliance Form-2)
  Use this form to document your efforts to secure DBE participation on your Bid, RFP, RFQ, or solicitation response. For questions concerning Good Faith Efforts, please contact our office at supplierdiversity@nola.gov. 
• Download DBE Participation Plan (DBE Compliance Form-3)
  Prime Contractors: Use this form to identify the DBE subcontractor(s) you will utilize on a City of New Orleans contract. 
• DBE Subcontractor Modification Request (DBE Compliance Form-4) Prime Contractors
  Please first contact your Compliance Officer to obtain this form and prior to any modifications/substitutions of any DBE subcontractor on the project.  This form is used to make a formal request to substitute any DBE subcontractor(s) you have stated you will utilize on a City of New Orleans contract.
