Get a Disability (ADA) Parking Zone

To request or remove a Disability (ADA) Parking Zone, the City of New Orleans requires the appropriate application to be completed and signed. If submitting by mail, the property owner's signature must be notarized.

 

To Request an ADA Parking Zone

Please submit the following:

  • A completed and signed ADA Parking Zone Request Application
    (Signature must be notarized if mailed)

  • A completed physician's certification form
    (included in the application packet)

  • A copy of your Louisiana Driver’s License

  • A copy of your handicapped license plate or placard

Please submit the following:

    (Signature must be notarized if mailed)
