The employee assistance program (EAP) is a voluntary work-based program that offers free and confidential assessments, short-term counseling, referrals for long-term counseling and follow-up services to employees who have personal and/or work related challenges at www.liveandworkwell.com .

What do you need to know?

• Who’s eligible to participate? The workforce!
  • All active employees, spouses/domestic partners, and dependents (household)
  • All outside agency employees enrolled into the city’s healthcare plan (household)
  • 24/7 Accessibility
  • Telephone, online and face-to-face support availability
  • Confidential support at no cost to employees
  • Retirees, spouses/domestic partners, retiree widows and dependents are not eligible.

Consultation

• Employees can speak with experts and specialists by phone and online as needed. Three (3) face-to-face sessions are included. Referrals to clinicians if issues require more attention beyond the 3 face-to-face sessions.
• Legal Services for personal legal matters (30 minutes of consultation by phone or in-person with licensed attorney) at no cost. Discounted legal services beyond consultation. Examples: Living Wills, Power of Attorney, Real Estate, etc.
• Mediation Services (30 minutes of consultation by phone or in-person) per issue per year at no cost. 25% Discount for ongoing mediation services. Examples: Child Custody, Collections, Family Disputes, Separation/Divorce.
• More services are available.

Consultations:

• Available 24/7 by phone
• No appointments are necessary for online/telephonic services
• All Services are confidential in accordance with federal and state laws. Personal records are never shared with your employer or anyone else without your permission.
• Exceptions
  • Except when required by law (court order)
  • Suspicion of child or elder abuse is reported
  • Threat of serious physical harm to any identified victim
  • If there is a concern about a member harming himself or herself
  • Gravely disabled member to the point of well-being being threatened
  • Member gives written consent
